Tony's balancing act gets tougher and tougher, and with Dr. Melfi in hiding, there's no one to turn to for advice.

Meadow begins acting out, as does Carmela, but in her own way – she's rethinking the sacrament of marriage. With Livia hospitalized (either recovering from a stroke or hiding from Tony), his long-lost sister Janice comes home. She claims she has only their ailing mother's interests at heart, Tony's big sister is still a Soprano and he's suspicious.

Things are no easier at work – for anybody. Uncle Junior's been indicted, so now Tony needs to keep a low profile. Pussy's acting surly; Christopher's distracted with filmmaking – and both men are about to face career-defining decisions.
